Schedule an Appointment
Immunizations are by appointment. Call (810)667-0448. We can schedule and discuss insurance and reduce cost options. Uninsured children are eligible for Vaccine For Children (VFC). Children will not be turned away due to inability to pay.
TB Testing
TB testing is done by appointment. You will also need to return 2-3 days later to have your test read and receive your proof of negative test
What to bring to your appointment
- Insurance card and ID
- Previous immunization record
- Parent or legal guardian must be present or Consent For a Minor To Receive Immunization Services
Vaccine Waiver Information
Parents or guardians opting out of the vaccinations required for their child or children to attend childcare or school, must request a waiver from their local public health department. Waivers are ONLY required for children in licensed childcare programs, preschool, kindergarten, 7th grade or new to a district. Waivers are available by appointment at 810-667-0448. Start early.
Travel Immunizations
Lapeer County Health Department does not provide travel immunizations.
