Children’s Special health Care Service (CSHCS) is a Michigan program to help families with a child or youth up to 21 years old, with a moderate-severe physical chronic conditions. It is not insurance, but it assists with eligible specialty doctor medical bills, etc. The Lapeer County Health Department nurses act as a liaison between the family and Lansing MDHHS-CSHCS Division. |

There are some forms that you might need through out your child’s enrollment in the CSHCS program. These can be filled out and emailed to the program or mailed to the contacts above.
